About the Role
We are seeking a Support Worker to join our team and help make a real impact in the fight to reduce, prevent, and end homelessness.
This role is focused on supporting individuals experiencing homelessness, particularly those with associated health needs, and working closely with the NHS to ensure smooth hospital discharges. Youβll play a vital role in empowering people to take control of their lives and work towards a more sustainable future.
Key Responsibilities
As a Support Worker, you will:

Develop and deliver a high-quality programme of support tailored to client needs

Promote choice, independence, and empowerment for service users

Address support needs and enable clients to achieve positive life changes

Work collaboratively as part of a team to achieve quality outcomes

Support clients transitioning from hospital discharge to safe accommodation
